Authorization Letter for Birth Certificate

Certificates play a major role in all spheres of life. Almost all the certificates one earns in life are important, but the most important one is the 'Birth Certificate'. It is not just a piece of paper, but a proof that a person has come in life. It is one the documents that you have to preserve it with you throughout the life. You will need it at the time of securing an admission in school, applying for driving license, visas, passports, opening a bank account, and obtaining a marriage license.


When a child comes to birth, the parents have to fill a form in the hospital. The form contains all the details of the child like names of the parents, permanent address of the family, name of the hospital, the child's biological information such as weight at the time of birth, height, gender, and exact date and time of the birth. The form will be authenticated by the doctor, who has done the delivery. Thereafter, the completed form will be sent to city's registrar office, where the actual birth certificate is prepared. A copy of the birth certificate will then be given to the parents by authenticating it and the original copy will be with a registry offices.

At the time of admission, the school will need the child's birth certificate to check whether he/she has attained the minimum age requirement for admission. When the child applies for a driving license, he/she needs to submit a birth certificate copy as a proof of age. If the social security number has been issued to them, they can submit a proof of the same. Alternatively, when you apply to open an account in a bank, or for a visa and passport, you have to submit an authenticated copy of your birth certificate. As a birth certificate is an important document in a person' life, some people try to forge the certificate to obtain a driving license, passport, or visas. It is for this reason, most of the country requires authentication of the birth certificate.

When you lose your copy of the birth certificate, you can apply to the registrar office of your city and obtain a copy. If you are not able to collect it personally, you can authorize someone by writing an authorization letter to collect the copy of your birth certificate for you. The authorization letter has to be written to the Office of the Vital Records. The letter should include the name of the person authorized to obtain the copy of birth certificate. It should have your full name and date of birth.
