Authorization Letter to Release Information

Sometimes you have to authorize bank, health care providers and insurance companies, or school and college to release your confidential information to a third-party, for some reason. The types of information you may need to release are credit, financial, medical, educational, and any other business-related issue.

When you want the bank to release your credit and financial information to a third-party for business or personal reasons, you have to write an authorization letter to the bank to authorize them to release your confidential information. This is because the banks are committed to maintaining the privacy of their customers. Same way, you have to write an authorization letter to the hospital when you want them to release your medical information to someone. The health care providers need this letter to follow the practice if the Health Insurance Portability and Accountability Act of 1996.

Authorization letter to release information is written to the particular department dealing in the type of information to be released. For medical purpose information, you have to write to the hospital administration when you are been treated. The letter should include the name of the patient and to whom the information is to be released. It should also give the reason for sharing the information with the intended recipient.

When you are trying to do business with a company, you have to have a good credit rating and sound financial status. The company would need something that can prove your worth and a decent record in the market. You can authorize your bank to release your credit information to the company through authorization letter. The letter to the bank should have your name and address and the type of account you hold with the bank. It should also include the details of the company/third-party to which you are allowing the bank to release your credit information.

This purpose of this permission is to assist in the diagnosis and treatment of my back injury. This permission, however, doesn't allow any other use of this information.
